Intermittent Androgen Suppression as a Treatment for Prostate Cancer: A Review.

چکیده
Prostate cancer continues as the most common malignancy in men in the United States, with a large number of patients presenting with advanced disease. The current treatment for metastatic prostate cancer, permanent androgen withdrawal, is palliative. Patients treated with permanent androgen withdrawal usually relapse and die secondary to prostate cancer's ability to progress to an androgen-independent state of growth. Based on preclinical studies and phase II trials, intermittent androgen suppression (IAS) appears to be a potential alternative to permanent androgen withdrawal. IAS may be a feasible alternative for the treatment of metastatic prostate cancer. In this paper, preclinical studies that form the basis of IAS and the clinical studies to date concerning IAS are reviewed. Through the cycling of reversible androgen suppression, there appears to be recovery of apoptosis and subsequent slower progression to an androgen-independent state. The small clinical studies conducted to date show the feasibility of IAS, and, in a few studies, there appears to be a survival advantage over historic controls. A prospective randomized trial which is currently under way will test IAS as a treatment alternative in stage D&sub2; prostate cancer.

A Mathematical Model of Intermittent Androgen Suppression Remedy for Prostate Cancer
Androgen suppression has been the principal modality for treatment of advanced prostate cancer for several decades. Although the tumor response rate to androgen deprivation is initially high, almost all patients relapse within several years due to proliferation of androgenindependent tumor cells. Since Bruchovsky et al. suggested in animal models that intermittent androgen suppression can prolo...
متن کاملCurrent Medical and Surgical Options for Androgen Deprivation in Prostate Cancer Patients
The growth of prostate cancer cells is driven by androgens. Thus, androgen deprivation, is one of the main treatment modalities in the management of prostate cancer. Historically, bilateral orchiectomy, which achieves 95% reduction of testosterone levels within 3 hours, was the only effective androgen deprivation therapy (ADT). In the 1980s, luteinizing hormone-releasing hormone agonists (LHRH-...

OBJECTIVES To review the literature and present new data of continuous androgen deprivation therapy (ADT) vs intermittent androgen deprivation (IAD) as therapies for prostate cancer in terms of survival and quality of life and clarify practical issues in the use of IAD. SUMMARY OBJECTIVES To review the literature and present a contemporary image of androgen deprivation for prostate cancer. METHODS We conducted a PubMed search on intermittent androgen deprivation. Articles obtained on intermittent androgen deprivation (IAD) and the experiences at Institut Montsouris were used for the review. RESULTS IAD is an approach to hormonal deprivation that holds effe...
متن کاملIntermittent androgen deprivation therapy in patients with prostate cancer: Connecting the dots
Intermittent androgen deprivation therapy (IADT) is now being increasingly opted by the treating physicians and patients with prostate cancer. The most common reason driving this is the availability of an off-treatment period to the patients that provides some relief from treatment-related side-effects, and reduced treatment costs. IADT may also delay the progression to castration-resistant pro...
